Chronic Care Management Program

Complications due to uncontrolled diabetes and chronic hypertension are among the top reasons why individuals visit the emergency department. To help reduce the reoccurrence of hospital visits, the Chronic Care Management Program here at the MASS Clinic focuses on providing consistent care for patients with chronic diseases such as Type 1 &2 Diabetes Mellitus and Hypertension. Patients who are enrolled in this program are seen by specific providers frequently to help reach their health goals. 

Through the results of blood sugar/blood pressure logs and hemoglobin A1c (HbA1c) blood test (3 month average blood sugar reading), the provider can assess and devise a plan to improve or maintain their chronic diseases accordingly.
