Chronic Care Management Program

Complications due to uncontrolled diabetes and chronic hypertension are among the top reasons why individuals visit the emergency department. To help reduce the reoccurrence of hospital visits, the Chronic Care Management Program here at the MASS Clinic focuses on providing consistent care for patients with chronic diseases such as Type 1 &2 Diabetes Mellitus and Hypertension. Patients who are enrolled in this program are seen by specific providers frequently to help reach their health goals. Through the results of blood sugar/blood pressure logs and hemoglobin A1c (HbA1c) blood test (3 month average blood sugar reading), the provider can assess and devise a plan to improve or maintain their chronic diseases accordingly.

Q. How do I become a patient?

To become a patient, you must meet the following criteria:
Be uninsured. This includes Medicare, Medicaid, or Share of Cost.

Be 18 years or older.

Have an income at or below 300% of the federal poverty guidelines.

Reside in Duval County.*

Q. Can anyone become a patient at MASS Clinic?
Muslim American Social Services
Anyone who meets our eligibility requirements is welcome. However, as a volunteer clinic, we cannot provide services for pregnant individuals, minors under 18, or offer STD testing, TB testing, or vaccinations.

Q. Are there any costs associated with the services?

MASS Clinic is free of charge for all patients that qualify for our services. Please note that pharmacies will charge for prescribed medications.
Q. Why do we need a proof of income (POI) statement?

Your POI is required for processing referrals. We cannot arrange specialty appointments without proof of income. The sooner we receive your proof of income, the faster we can proceed with your referral, which generally takes about 6-8 weeks.

Q. Do you administer vaccines?

No, but the Florida Department of Health can administer them at a nominal cost.

Sundays from 1:30 PM to 3:30 PM and Tuesdays from 9:30 AM to 11:30 AM. You may visit up to two times a month.
